Angels of Grace Foster Family Agency was recently awarded the "Homeless Prevention Rapid Re-Housing Program (HPRP) 2009 grant to expand current services in serving the homeless.  The name of the program is called the "Angels of Grace Homeless Prevention Program (AOG HPP)."  The target population is to focus on females between the ages of 18-30 with or without children up to 18 months.  We are are also currently serving males, single, separated, or married couples with assistance 18 and older for up to 3 months.  We are also curreWe will work in conjunction with the County and City of Fresno.

Our mission for the AOG HPP is to provide safe and secure housing for individuals and families who are homeless or at-risk of becoming homeless.  The ultimate goals is to do all necessary to prevent individuals and families from experiencing homelessness and quickly re-house and stabalize these individuals and families. 
 
 
AOG HPP offers the following services to the public:

* Agency Referrals:  If the participants are in need of assistance in the areas of mental/physical disabilities, substance, alcohol, or drug abuse of any form, AOG HPP can make referrals to other outsourced agencies on behalf of the participant who are interested in seeking help.
 
* AOG HPP Medium-Term Rental Assistance (4-18 Months):  The AOG HPP Medium-Term Rental Assistance Program is an income-based program, thus the participants must be certified to participate in the proram.  To do so, the participant will be required to complete an Income Verification Form for all family members over the age of 18 residing or will be residing in the household during the 4-18 month program.  The participant in this program must complete the "AOG HPP Authorization Application"  To apply for the AOG HPP Medium-Term Rental Assistance Program for HPRP Funds, please click here.  

 
 
* AOG HPP Short-Term Rental Assistance (0-3 Months):  The AOG HPP Short-Term Rental Assistance Program is an income-based program, thus the participant must be certified to participate in the program. To do so, the participant will be required to complete an income verification form for all family members over the age of 18 residing or will be residing in the household during the 0-3 month program.   Anyone interested in applying for this program must complete the AOG HPP Short-Term Rental Assistance application with an AOG HPP representative.  To participate in this program, the participant must complete the "AOG HPP Authorization Application." To apply for the AOG HPP Short-Term Rental Assistance Program for HPRP Funds, please click here.

Complete an Income Verification Form for certification and complete all required documents before final approval.  

Ø

Obtain approval of funds from an AOG HPP representative and present this amount to the rental property manager.

Ø

Speak to a financial resource specialist who will ensure that the participant can indeed afford the living space they are seeking to occupy.

Ø

Participate in Case Management/Wrap Around Services for the duration they are in the program.

Ø

Must re-certify every three (3) months for this program.  Please note that this re-certification process will not disqualify you from participating in the program, but will help determine your cost of rent and utilities.

* Case Management/Wrap Around Services: If participants enroll in the AOG HPP Short-Term Rental Assistance Program in which AOG HPP would assist the participant with rental assistance for a term of 0-3 months; or the AOG HPP Medium-Term Rental Assistance Program in which AOG HPP would assist the participant with rental assistance for a term of 4-18 months, these participants are required to work with an AOG HPP Case Manager.

* Credit Repair/Other Financial Assistance Services:  AOG HPP will assign desiring participants a financial consultant who will help the participant with settling debt, budgeting income and expenses, or outsourcing to other financial institutions for further assistance if necessary.  To participate in this program, the participant must complete the "AOG HPP Authorization Application." To apply for the Credit Repair/Other Financial Assistance Services for HPRP Funds, please click here.
 


* Move-In Cost Assistance:
Through this service, AOG HPP will assist participants in move-in cost for their desired living facility.  Participants will have the option upon applying for other sources of available funds for this program or the AOG HPP Short-Term or Medium-Term Rental Assistance Programs.  Participants will be required to complete an income verification worksheet and supply necessary documents to qualify for this type of assistance.  This assistance is provided to one individual or family once a year.  To participate in this program, the participant must complete the "AOG HPP Authorization Application." To apply for the Move-In Cost for HPRP Funds, please click here.
 
 


* Security Deposit Assistance:
  Through this particular program, interested participant will receive a certain percentage of funds (depending upon income) to help on security deposits to move into a rental property. The AOG HPP Security Deposit Assistance Program is a voluntary program; however, participants are expected to follow all rules of AOG HPP. If a participant fails to follow the rules, they are eligible for grounds of termination from the AOG HPP Security Deposit Assistance Program. Applicants may also apply for other programs if necessary such as the “AOG HPP Short-Term Rental Assistance, AOG HPP Medium-Term Rental Assistance Program, Move-in Assistance, Utility Deposit/Payment Assistance, or Credit Repair/Financial Assistance.”  To participate in this program, the participant must complete the "AOG HPP Authorization Application." To apply for the Security Deposit Assistance for HPRP Funds, please click here.

* Utility Deposit and Payment Assistance Program:  AOG HPP will provide participants with funding through HPRP for the use of Utility Deposit or Utility Payment Assistance to assist participants who are at-risk of losing their utilities for the lack of funds. The Utility Deposit and Payment Assistant Program may be used to pay gasoline, electric, and water services. Participants may not use HPRP funds to pay past due utility bills. The funds will not be paid to the participant, however, but will be paid directly to the utility company to start services or pay bill for the participant at the rental property for the participant. The participant may apply for both the Utility Deposit and Utility Payment Assistance Programs. If other services are needed, the participant may apply for other HPRP funds through AOG HPP in conjunction with these programs.  To participate in this program, the participant must complete the "AOG HPP Authorization Application." To apply for the Utility Deposit and Payment Assistance Program for HPRP Funds, please click here.

 
 

Required documentation to participate in the programs listed above:

 Birth certificates of all persons living in the household  To obtain a copy of your birth certificate, please click here,

 Check stubs for the last 2 months for income verification,

 If the participant has a checking and savings, bank statements for the last 2 months,

If the participant receives food stamps, TANF, or any other type of public assistance, the participant is required to provide AOG HPP with copies of the these documents. If receiving Section 8 or Public Low-Income Housing, the participant must provide authorization from the Housing Authority Representative of approval to participate in the AOG HPP program for other financial assistance.

PLEASE NOTE:  Failing to provide all required documentation on a timely basis may cause a delay in the application process, which can result in ineligibility of AOG HPP Security Deposit Assistance Program and denial of the application.

HPRP Guideline Eligibility:
  • HPRP guidelines requires that participants meet the three main eligibility criteria to be able to receive HPRP Funds for either financial assistance or home stabalization:

Any individual or family provided with financial assistance through HPRP must have at least an initial consultation with a case manager or other authorized representative who can determine the appropriate type of assistance to meet their needs.

The household must be at or 50% of Area Median Income (AMI).  Income limits are update yearly  and can be found at http://www.huduser.org/DATASETS/il.html. All participating agencies will use HUD's Section 8 Income Standards for HPRP.
Ø
 
 
 
The household must be homeless or either at risk of losing their housing and meet both of the following circumstances: (1) no appropriate subsequate housing options have been identified; AND (2) the household lacks the financial resources and support networks needed to obtain immediate housing or remain in its existing housing.

AOG HPP Eligibility Crieteria:
  • Participants must complete the required applications for the desired program, the "Income Verification Form" for the certification process, attend the AOG HPP orientation and intake process.
  • Participants must be emanicipated females aging out of the child welfare system between the ages of 18-24 with or without children.  Participants will have the option of living in one of the AOG HPP pre-arranged living facilities or an AOG HPP representative will assists the participant seek other outsourced living arrangements.
  • The AOG HPP programs are income-based.  Participants applying for any types of services through AOG HPP must complete provide all necessary documentation and provide proof of income to participate.  Everyone living in the household over the age of 18 must provide proof of income.  AOG HPP will not assist participants with living arrangements that is out of their means financially.  If this type of request occurs, an AOG HPP representative will assign the participant to and AOG financial counselor to discuss further options and determine the best living situation for the participant.
  • Participants seeking to apply for the HPRP Funds through AOG HPP must be very low-income at 50% rate is $19,550 for 1 person to $36,850 for 8 people; for extremely low-income at 30% rate is $11,750 for 1 person to $22,100 for 8 people; and for low-income at 80% rate is $31,250 for 1 person to $58,950 for 8 people. (See the Fiscal Year 2009 Income Limits Documentation for AOG HPP Participants below). Please note, not having any income will not disqualify you from the program. See chart below.
  • Participants must notify AOG HPP within five (5) business days of a change in income, Section 8, Public Low-Income Housing, or public assistance.
  • Because the participants rent is income-based, the participant must resubmit proof of income every three (3) months for rent re-calculations.  Please note, the re-calculations of rent will not disqualify the participant from remaining on the program.  It will, however, help the AOG HPP representative determine the participants cost of rent.

How to Apply for HPRP Funds through Angels of Grace Homeless Prevention Program, please do one of the following:
 
  • Interested participants must complete and return the application for which they are applying to an AOG HPP representative by fax at (559) 268-7728, in person or by mail at Angels of Grace Foster Family Agency at 2211 N. Fine Avenue, Fresno, CA 93727 at the corner of Van Ness and Toulumne, across from the Fresno Housing Authority and CVS Pharmacy (use to be Longs Drugs).

  • A referring agency may also complete and submit a referral form on behalf of the participant. Upon this type of request, an AOG HPP representative will be in contact with the participant within five (5) days of receipt of the referral and schedule an appointment with the prospect participant informing them of their referral by the referring agency to the AOG HPP.
  • The interested participant must provide income verification for the previous 90 days and or verification of ALL assets. The application process is not complete until the participant has provided all the necessary documents for complete verification.

Other Available Resources – HUD Low Rent Assistance Program

Participants who need further assistance may go to the U.S. Department of Housing and Urban Development (HUD) Website for other rental assistance for public housing, located at:

  • Low-Income Apartment Search: The low-income apartment search criteria, supplied by HUD, offers participants the ability to conduct and apartment search for 1 to 5 bedrooms or more for privately owned subsidized housing. HUD provides funds to the property manager directly to reduced rent normally paid by participants. The low-income apartment search criteria can be located at http://www.hud.gov/apps/section8/index.cfm. This can also assist senior citizens and disabilities in apartment searching.
  • Housing Choice Vouchers (Section 8): The Housing vouchers, Section 8, is used by very low-income families, elderly, or those with disabilities for apartments, single-family homes, or townhomes. This program, provided by HUD is administered by the Public Housing Agency (PHA), supplied through federal government funding. The participating families are required to find suitable housing as explained by HUD and PHA that meets the minimum standards of all health and safety regulations. Eligibility for the vouchers are based on the families gross income and family size, to US citizens and specified non-citizens who have eligibility. Interested applicants may apply for a voucher by contacting their local PHA provider or HUD office at http://www.hafresno.org/.
